EMR - Electronic Medical Record - typically owned by institution (provider or payer), focused on interactions involving paid services

EHR - Electronic Health Record - broader data set, still owned by institution (hmm, some frame it as being the multi-institution version, like a RHIO)

PMR - Patient Medical Record - like EMR, but controlled by patient

PHR - Patient Health Record - controlled by patient, most likely to be broadest in data capture (Diet And Exercise logs, etc.)
Does any of it really reduce Medical Error?
